Recommendation for filing the complaint

 

1. Use polite words
2. Specify the facts or any behaviour of the State agency, including any concise details relating to the information
3. Statements that must be prescribed in the complaint
    1) Particulars and details of the complaint maker; comprising
        - Name and family name, address or such other place of contact and telephone number
        - Date of filing the complaint
    2) Particulars and details of the State agency and the act of violation of law of such State agency
        - Title of the State agency
        - Facts or behaviour of the State agency which is considered violation of law or the State agency’s failure to comply with the law.
    3) Any supporting documents (if any)
        Should there be any supporting documents such as the request for the information, an objection to disclose information, such

    documents must be attached. If the complaint lodger had attached these documents to the complaint, it would be helpful for the

    Board in considering the complaint in due course.
4. Avoid using any impolite, sarcastic or insulting language, or the language referring to any third party in the complaint.
